Transportation Survey to Residents of Clare, Isabella, Lake, and Mecosta County

This survey is a chance for you to tell us how transportation access impacts you and your family. The North Central Community Health Innovation Region, a collaborative body that brings together cross-sector partners including local health departments and hospitals across the 10 counties of Northern Lower Michigan, is working to improve the health of communities in these counties by collecting data, identifying key issues, and bringing people together for change. 


Do you have access to transportation? What resources and strengths does the community have that can be used to improve transportation access to health services?


The counties within this region include: Arenac, Clare, Gladwin, Isabella, Lake, Mason, Mecosta, Newaygo, Oceana, and Osceola. For this project, we are focusing on Input from Residents Living in Clare, Isabella, Lake, and Mecosta County.
